If one thing is clear in reproductive medicine, it is that each patient is unique and, as such, all treatment must be completely personalized and adapted to the characteristics of each case. Today we are going to talk about uterine surgeries: what do they consist of? How are they carried out? In which cases are they recommended? What can a personalized evaluation influence to detect whether they are necessary or not?

What is a metroplasty and in which cases is it necessary?

The uterine surgery or metroplasty It consists of the surgical correction of the walls of the uterus in those cases in which the uterine cavity is not anatomically normal, which in many cases makes it difficult and prevents the implantation of the embryo, necessary to achieve a pregnancy. It is carried out by hysteroscopy, introducing an optic through the cervix. It is generally performed on an outpatient basis under anesthesia and has a very low complication rate.

Various studies published in prestigious scientific journals have shown that hysteroscopy is not required routinely in all patients who are going to carry out an in vitro fertilization cycle (whether this is the first or not).

Precisely, and as a first screening measure, at the IVI Valencia clinic a 3-dimensional uterine ultrasound is performed in consultation on all patients to obtain information on the anatomical shape of the uterine cavity. Thanks to the experience of the Ambulatory Surgery Unit of IVI Valencia and the collection of data from all those thousands of women who have already come to our consultation, we can detect in which cases surgery would be necessary because they need a correction and in which cases it will not be necessary for the patient to undergo surgery.

If the 3D uterine ultrasound shows a correct uterine cavity, our specialists will assess the performance of a consultative hysteroscopy. In these cases, when the objective is simply to visualize the endometrial surface and rule out pathologies at this level, it will not be necessary to undergo surgery or the use of anesthesia.

However, in those cases in which the uterine cavity is not adequate by 3D ultrasound, hysteroscopy is planned directly in the operating room, since in this way the metroplasty and an evaluation of the endometrial mucosa can be performed in parallel.

But, as we mentioned, the key is in the individualization, in knowing which patients can benefit from this type of intervention and which cannot.

On the other hand, 3D ultrasound can also be useful for the detection of malformations that may be making it difficult to achieve a pregnancy, such as the T-shaped uterus.
